Mission of Community School Health Center:

The UICNI-Division of Community Health School Community Health Center provides comprehensive health care as a part of a school-based health program funded by Healthy Schools/Healthy Community’s Bureau of Primary Health Care, and the Department of Human Services.  The health centers:

  • Coordinates resources of a UIC NI School Community Health Center, Host School, and University of Illinois at Chicago Medical Center resources.

  • Provides comprehensive health care for chronic and acute illnesses, health maintenance services, and preventative care, education and counseling.

  • Supports formal links and referrals to Miles Square Health Center and the University of Illinois at Chicago Hospital for emergency care, specialty referrals, and diagnostic services.

  • Provides community health promotion and education in the areas of nutrition, health maintenance, and healthy lifestyles.

  • Provides case management follow-up for illness-related absences.

Community School Health Center Advisory Board Purpose:

Advisory Board Purpose

The Community School Health Center Advisory Board is formed for the purpose of shaping, guiding, and supporting the Community School Health Center in achieving its mission within the school and health center.

Governance Structure

  • The Advisory Board includes representation from the University of Illinois at Chicago, Neighborhood Initiatives-Division of Community Health (UICNI), University of Illinois at Chicago Medical Center, Chicago Public Schools, host school’s administrators/staff, students and parents.  It consists of approximately 10-12 executive members, with provisions for other board members to participate in subcommittees
  • Attendance at the Advisory Board meetings is mandatory for the following CSHC staff:

    • Program Director

    • Medical Director

    As Needed Basis:

    • Ancillary Support Service Members

  • The Advisory Board provides guidance to the UICNI in the operations of the Community School Health Center, but does not have authority to enforce its recommendations.  The Advisory Board offers guidance on:

    • All planning activities

    • The service plan for the Community School Health Center

    • New programming and initiatives

    • Written materials and products of the Community School Health Center

    • Community School Health Center marketing activities

    • Allocation of new resources

    • Community School Health Center budgets

    • The Advisory Board meets regularly throughout the school year.  The Community School Health Center’s Program Director, with the approval of the UICNI, coordinates its activities.  Changes to the frequency is be determined by the board.
  • The Advisory Board roles may be rotated among the membership, or voted on by the Board.

  • Members are encouraged to commit to two years of service for purposes of continuity.  Membership is limited to serving a maximum of four years.
  • Other Community School Health Center staff participate in the meetings when appropriate.
  • A separate Youth/Teen Advisory Board is optional, with representatives from all grades and a broad cross-section of students represented.  The Youth/Teen Advisory Board focuses their efforts on providing guidance to the Community School Health Center and developing marketing activities.  A representative from this Board serves on the regular Advisory Board.
